This outfit has nothing to do with my call weekend. I did not wear sparkly pants to the hospital. Just sharing something pretty during a typical not-so-glamorous call weekend.

Woohoo! It's Sunday and after 4p.
I want to thank my colleagues for awesome teamwork this weekend. I could not have done all of it by myself... I could have but I would have been up all night long... I would have been that physician looking like a zombie with lack of sleep. I would have been super grumpy.
 
But that was not the case! woohoo!

on call again?



i am back... continuing the day in the life of a resident with photos of my outfit splattered throughout this post used as a "page break".

for part I, head here... I talked about my big program and what short and long calls mean and changes in the work-hour/week limit that occurred while I was in the middle of my training.



Next, I will outline the different resident roles when "on-call"where I trained.

Here's the call structure.

There is a SENIOR-IN-HOUSE. This is an upper level resident who are post-graduate 2 all the way to 5. There are peds residents who signed up for the 5 year plan like the Triple board residents and Pediatrics/Emergency residents. {I signed up for the 5 year plan too as a Child Neuro resident.}



Next is SENIOR ASSIST. Like the term implies, this is an upper level resident who backs up the senior-in-house. "When I was a resident," the senior assist was the back-up for Heme/Onc. Heme-onc patients are on the sickest in the hospitals (besides those at the Peds ICU). I have had to call my senior assist twice when i was taking call for Heme Onc as an intern.


The other peds upper level who were on-call were at the ICU. We have two ICUs in our hospital. we called them PICUNorth and PICU South (they have changed their names now). PICUNorth admits were of general PICU cases. PICUSouth patients are the CV Surgery pts and  some general Peds ICU patients.



Besides Peds upper levels taking call at the Peds ICU, we also had Emergency medicine residents and Med/Peds residents who were part of the call pool. Peds ICU training is part of their training curriculum.
















                              







For the intern call structure, we used a color system to designate roles.
"when I was a resident," we had a GOLD intern, SILVER intern and PLATINUM intern.

You'd want to BE the platinum intern because cross coverage was not too bad for those interns... plus they are "back-up" interns doing admissions.

Gold interns covered: Developmental Pediatrics,  Cardiology, Metabolism and Genetics
Silver interns covered: Hospitalist, Infectious Disease, Pulmonary
Platinum interns covered:  Renal, Endocrinology

{I don't remember who covered Gastroenterology}


If a resident is on-service for a specific specialty,she will cross cover her own team and also the rest of the patients in that color grouping.

Heme onc interns covered just Heme-onc patients.
NICU residents just covered NICU patients.


{but as I have heard, the roles have been changed for the color groupings}


Neurology and surgical subspecialties were not included in the Peds call pool. Those specialties had their own coverage and resident call pool.


There's also residents in the "jeopardy call list". That's where the back-up residents are chosen from when a resident is sick, etc and a need /call night/ day-time coverage has to be filled. These residents in the "jeopardy call list" are usually on their electives or outpatient months or research months.

















We had a sort of "graded" call system.
I was "q4" meaning every 4th night I was on call as an intern. My friend SP and I were the "lucky ones" (I'd remember this FOREVER!) but we would be the "lucky" ones to get 9 calls in one month! That was when the rotations were based on calendar months rather than 4 week blocks. My training program has now adopted the 4-week block. Therefore length of experience is similar with different rotations or subspecialties.



As I became a second year in peds, my calls went to "q5" meaning every 5th day I was on call for busy services -- which means peds Hospitalist, Peds ICU. The rest of the services were as great as q6 (every 6th night!).




ok.. I am stopping there for now. It's getting boring. You might find this interesting if you are thinking of going into medicine or looking into different pediatric residency programs.

booties for work and play {wiws}

Let's start with the negatives this weekend:
  • I spilled coffee on myself two days in a row! I was being a good co-worker and brought coffee to my resident Saturday morning. I forgot the green prevent-spill cover. I got out of my car and juggled 2 coffee cups, work bag and morning treats -- and coffee spilled inside my right arm sleeve! good thing, the coffee was not burning hot anymore.
  • I spilled coffee on my white coat while rinsing my coffee mug. White coats are  not just for covering against vomit and other germs.... apparently for water faucet explosions too. 
  • This was my black weekend.... I was on-call both Sat and Sun.
  • Because of DST, I had an extra hour of call coverage.
  • And I work at a hospital -- there are a lot of sick kids (no surprise there).
  • Sebastian had a diaper explosion and I missed Communion.

  • I still got out of the hospital around noon both days! yippee!
  • I was not first-call so i got some sleep! The pager did not go off from 10p-7a both nights. (high five!)
  • I did not have to sleep at the hospital.
  • I asked for a treat both days -- qdoba for Sat lunch and sushi for Sun lunch. My wish was granted by hubs. 
  • I went to Mass with my family.
  • And at 5p -- I am not on call anymore.

Still pregnant, letter, pager blues {7 quick takes Friday vol 40}

1.
I'm still pregnant. I'm 37 weeks plus change so I'm considered full term. I'm actually feeling pretty well. Walks after a full day in the clinic are more tiring. Hips are achier. Bladder emptying trips are more frequent. But overall, I'm doing well.

2.
Remember my request for prayers in a previous 7qt post? Thanks so much to all my prayer warriors. I've been less anxious and in fact, excited about this challenge instead of freaking out. Same requests this week: trust in God and peace that it will all work out.

3.
I received my first letter from the teacher regarding my TK's behaviors. Sad, sad, sad day. I'm actually waiting for the letter because listening is not my TK's best skill. Hubs told Lil D: mummy and I don't want to receive any more letters from the teacher, ok.





4.
I'm on home call this weekend. What was I thinking when I made that schedule?! Obviously not the fact that baby has dropped and that I'm going to the bathroom every 1.5 hour and that a snail has a faster pace than me!

5. More pregnancy takes:
A mom in clinic told me today:
Have a good Labor Day. Don't take it too literally and go into labor! (Funny!)
I don't think whoever I'm on call with {staff or resident} will appreciate me missing in action if I don't take my last 2 weekday calls at 38weeks + 2 days and at 38+ 6 days. Again, I was a bit over ambitious to take as much home call now. my rationale was when I return to work, I won't have a weekend call until December or tons of weeknight calls my first two weeks back.

6.
Me: this chair feels wet.
Co worker looked at me visibly concerned.
Me: my water did not break, no worries.
Coworker: a comment like that coming from a full term pregnant lady is not really something we take lightly.
I'm distressing my coworkers and parents in clinic. I think they expect me to go into labor and deliver then and there.... I don't plan on doing that. But who am I to plan these unpredictable things?

7 quick takes vol 36

Let's join Jen for our Friday party!

1. Call night
I had the best call night ever last Monday. No beeper noise after 9p. Yet it was a tiring night! I Had a post call headache the next day that required Tylenol.
Reason: my daughter. She tossed and turned all. Night. Long. I could not sneak out of her room. So yeah, I spent the night in her room,sharing her twin bed. It was not my pager that woke her up -- because it was a quiet pager that night.


2. Old friends.
Don't you just love catching up with old friends?
Those long conversations on the phone?
I should do it more often. It definitely relieves stress....


3. Recipes I wanna try:
Two posts this week that captured my attention. I'd want to try this chicken cordon bleu nugget recipe from sixSistersStuff. I have picky eaters here at home and I think this will be a winner. Daniel loves chicken nuggets, cheese and ham! So why not mix them all together?!

Martha also posted this recipe -- nom, nom. I love easy-to-folow recipes. I think this is going to be full of flavor and I cannot wait to try it. It won't be a hit for my picky eaters but the grown ups in the D household will surely love the shrimp!

4.mama list
Daniel is starting preK at our parish school. Mornings are stressful --- mostly for me because I tend to push it to the limit... I am an alarm snoozer and squeeze as much sleep as I can to the last possible minute. But end product usually is rushing in the shower, rushing to get my breakfast and lunch ready and so on and so forth.
I don't want to be as stressed anymore. I'd be the primary driver for my son as my hubs take Isabel to the east side. I've been lucky that Derrick drops off the kids so I can go to clinic or the hospital on time for the past 4 years.

I Found this list from thepleatedpoppy blog.
I think it will help me a lot. I like planning and organization. I lay out all my work clothes and accessories the night before. When I'm pumping/nursing, I place all my pump accessories and new bottles in my pump bag before i go to bed. I usually pack our lunches the night before and arrange all necessary breakfast items. Still I just can't make my coffee the night before!

My next to-do is search for healthy preschooler snacks and lunches. Any suggestions? I've got a very, very picky eater.


5. State Fair
Derrick and I started going to the state fair the first year we started dating. It has become a tradition! We only missed one year -- in 2009 when Daniel was only 3-5 weeks old. I did not want to bring my newborn to a crowd-filled state fair! I was a new parent who was scared of everything... Pretty much. We used to go once...then some years, we have gone about two-three times.
We love fair food! I don't eat it everyday so i splurge. I pay for it afterwards because of the all the grease and fat. Haha... The corn in the cob, the milkshake from the milk barn, the pineapple smoothie, the elephant ears... O so yummy!
Kids love seeing the animals -- the biggest boar in the swine barn, the cute rabbits, different breeds of chickens, sheeps and horses. Daniel loves the tractors. He basically rides every single one! There's also free concerts and activities.
We are definitely excited! The state fair opened today and we are gonna go! Soon!


6.Medical Mondays
I love link ups. I found great blogs because I've "linked up". Medical Mondays is celebrating its 1 year anniversary! Join us on Monday August 5th if you're in the medical field, married to somebody tied to a pager, med student, premed student etc!
I'm gonna work on something about residency and breast feeding/pumping {yes, both can be done!!!} so come back next week.


7. Picture I am sharing




Isn't this just so, so precious? My good friend Sister Solanus Casey is visiting home for two weeks. She's Daniel's godmother. She's one of my oldest friends here in the US... Have known her since she was a junior in HS and I was a freshman at IUPUI.

She showed her crucifix to Isabel and said: this is Jesus. Do you want to give him a kiss? My little girl did not hesitate... Oh. So. Precious.

White and blue {wiws}

Let's start with Mass behaviors:

The beginning: I was rushing to get ready for Mass. I took a nap and woke up less than an hour before the bells start tolling. So as I rushed to shower and dress, I got paged. Oh yeah, I'm on call this weekend.

We made it in time at the last note of the entrance hymn. Ok so far so good. I even put my lipstick on. No face powder. No mascara. No blush. But I got my hot pink lipstick on.

End of the first reading, I felt the pager go off on vibrate on my pant waist. Great.
Time to be a doctor. So Isabel and I went outside. She was entertained by the squirrel while I answer the hospital call.

Now back to Mass. We heard the last third of the Gospel. And I should be grateful as I heard the entire homily. Fr. Roberts talked about our battle scars for Christ. Like St. Paul enduring all those hardships, shipwreck, and beatings all for Jesus.

Then another vibrate coming from my waist.

Ok back outside. I took Isabel with me as she's getting antsy anyway. Unfortunately the squirrel left its tree so she got bored. She started banging the side door calling 'daddy'. Fortunately, the page was a 2 in 1 call.

Inside the church, I made it to the Eucharistic prayer and proclamation of the mystery of faith. At this point, I was already losing it and falling apart inside. My Mass participation was in bits and pieces. The kids making a fuss was getting to me. The concentration is gone. I prayed and prayed asking for grace.
I might have missed a good part of Mass but I still received Holy Communion. I couldn't bear the thought of not receiving Christ and his grace for another week. How can I survive Mass or homecall or the coming week without God???

Then I just found out after dinner that I wore my cami backwards. That's why the front (which is technically the back) was 'riding up' my belly. Oooops.
